What does a telephone handset symbol with keyboard, near the battery symbol in right hand top corner of iPhone 4 indicate?

That TTY is on. Go to settings>phone and go down and you will see that the TTY setting is on. If you turn it off it goes away-----
TTY is enabled ( Settings > General > Accessibility I believe).
